Registration Information

Take advantage of the reduced registration fee by registering early on or before Friday, May 8.
Fees are quoted in both Canadian and U.S. dollars.
Fees include the Canadian government's 7% goods and services tax.
Fees apply to all those attending and/or presenting at the conference.
Registrations received after Wednesday, May 20 will be processed on site.

Early Conference Fee (Deadline May 8)- $300 Cdn or $216 US
Late Conference Fee - $325 Cdn or $234 US
Student Fee (with valid student card) - $125 Cdn or $90 US
Late Student Fee (with valid student card) - $150 Cdn or $108 US

Non-CAIMS/SCMAI/SIAM Members
Early Conference Fee (Deadline May 1)- $340 Cdn or $245 US
Late Conference Fee - $365 Cdn or $263 US
Student Fee (with valid student card) - $140 Cdn or $100 US
Late Student Fee (with valid student card) - $165 Cdn or $118 US

The conference registration fee includes

all conference sessions
Thursday (May 28) wine and cheese reception
Saturday (May 30) conference banquet
Abstract book
Refreshment breaks
7% goods and services tax.

The non-member fee includes CAIMS/SCMAI membership of $40 Cdn regular/$15 Cdn students.

Extra Banquet Tickets (for guests) are available at $75 Cdn each or $ 54 US.

Accommodation and airfare transportation costs are not included in the fees.


How to Register

By mail
Complete and mail your registration form to SFU Conference Services. Please submit a separate registration form for each person; blank registration forms may be photocopied.

By fax
Use your Visa or Mastercard and fax your registration form/s to (604) 291-3420.

Student Fee
The student fee is offered to graduate and post-doctoral students. Students must provide a copy of their student ID card or submit proof of full time status.


Methods of Payment

You can pay by cheque, Mastercard or Visa. Cheques should be made payable to Simon Fraser University and accompany the registration form. SFU does not accept American Express.

Registrations will only be processed with fee payment or a purchase order number from your institution (Canada only).

All credit card transactions will be processed in Canadian dollars and are subject to current exchange rates.


Cancellations and Refunds

Full refunds, less a $40 Cdn or $30 US processing fee, will be granted to cancellations received by May 20, 1998. No refunds will be issued after that date.

To obtain a refund, please return the original SFU receipt (not a copy) to SFU Conference Services. Refunds will be mailed after the conference.


Receipts

You will be sent a receipt as confirmation of your registration. Any receipts not mailed out by May 15 will be available for collection at the Conference Registration Desk.
